Tools & Tech for your Success

8 week program (plus 2 months post program supports) that will provide persons with intellectual disabilities/special learning challenges access to supports needed to enhance social determinants of health, including food security, mental health supports, and housing services. Program includes a mix of learning activities, one-on-one coaching and supports, and other participant engagement opportunities. Access to a referral network will be available based on the specific support needs, including health services, financial management, food security, mental health supports, and housing services. Participants without access to technology (hardware and internet) will be equipped with a tablet and an internet connection for the duration of the program.

HOPE (Heading to Opportunities in Pre-Employment)

12 week program (plus 2 months post program support) designed to support youth (18-29) in receipt of Ontario Works stabilize their situation(s), establish new bearings and to successfully navigate to next steps post-COVID crisis. Program includes intake and assessment, workshops, webinars and one-on-one support.

 

We Care – Integrated Wrap Around Support for You

12 week program (plus 2 months post program supports) for adults 45+ who are marginalized. Program will take a holistic, integrated and coordinated approach to support participant stability and well-being. Program includes one-on-one and group activities to address social isolation, physical decline, and psychological pressures. Participants will receive support to access food and essentials, to manage parenting issues and to access financial benefits.

 

Innovative Case Management Program: 2-Spirited People of the 1st Nations

12 week program for Indigenous individuals and families using a holistic, person-centred approach that includes three culturally appropriate service streams. Each stream is designed with a unique purpose, approach and delivery style to provide mental health support in the following three service streams: Individual and Family Counselling Supports, Group and Peer-Led Supports and Foundational Life Skills Development.

 

Innovative Case Management Program: Mental Health Support for Black Individuals and Families

12 week program for Black individuals and their families to improve access to mental health services through timely, culturally safe and responsive mental health supports. This program has three streams: Individual Mental Health Support, Group Support, and Support with Life Skills Capacity Building.

 

Innovative Case Management Program: Overcoming Trauma

12 week trauma-focused program that integrates a variety of evidence-based treatment perspectives. This program will help participants gain a deeper understanding of the impact of trauma on their current lives and gain effective strategies to manage the symptoms of trauma. The program is designed to create a safe space for participants to engage in self-exploration and receive support and encouragement from each other.
